Person Centered Care

Where the “Person” is the center of the plan.

DS Heavenly Haven provides two forms of residential services that range from 24-hour care in a group home to semi-independent living with supports. Each individual referred to DS Heavenly Haven receives an assessment to determine which level of care is appropriate. Some residents will may require higher levels of staff support and supervision, while others will progress to lower levels of care. It is our goal to assure all residents live in the most independent setting possible.


Group Homes

Currently we have 4 licenced group homes in the Owosso area and are expanding.

It is the goal of every staff at DS Heavenly Haven to give our residents the opportunity to live as independently as possible.

Each group home is centered around the uniqueness of the individuals that reside there. Each individual has their own bedroom decorated to their tastes. Each home has been made to be warm and inviting, a place you can truly call home.

The group homes are staffed 24-hours a day. Trained direct-care staff provide many services throughout the day and night at each Group Home. They are required to do all med passes (must be state-certified), healthcare checklists, daily schedules, skill training programs, medical appointments, transportation, recreational outings, leisure activities and everyday living skills.

Semi-independent Homes

Semi-independent homes are available for individuals that may not need as much direct care, but still need housing and some supports, which may include medication management, transportation, etc.

The semi-independent homes have staff available every day. Care levels vary, but generally semi-independent residents do not require 24-hour care. Many of the individuals living in a semi-independent home are employed or spend time involved with community volunteerism or other daily activities.
